Equipment Rental Agreement

By signing below, I hereby assume all and full responsibility and liability for any damage that may occur to said weapon(s)/equipment while in my possession. If the items are damaged in any way or fail to function while in my care, or I fail to return said items, I authorize CQB City, LLC to charge my credit card listed below for the full retail cost of said them. I also acknowledge that I am in no way entitled to any item that is returned damaged and that it will remain in the custody of CQB City, LLC.
